Care that starts from the person.
A web platform on AWS for AICP.COM, the NextGenerationEU project in which Lares and Fundación Pilares move 24 care homes and home care services in 8 municipalities towards person-centred care. It collects and shows the results of the quality assessments.
The platform is built in two parts. The first runs self-assessment in the care homes, with the standards behind it, the training and the improvement plan that comes out of every assessment; the second serves home and community care, with territory coordinators, case managers, and a life history and life project for each person. The programme reaches 954 people in 24 care homes and 240 at home.

Show
The results are gathered on AWS and shown back to the teams.
